August Performance PT is a cash-based practice. We do not bill insurance directly. We provide superbills after each session that you can submit to your insurance carrier for potential out-of-network reimbursement. HSA and FSA payments are accepted.
No referral needed. Ohio is a direct-access state, which means you can book an evaluation with Jordan without a physician's order. Just reach out and we'll get you scheduled.
We'll follow up within one business day to schedule your free discovery call. That call is a chance to talk through your situation, ask questions, and figure out whether we're the right fit before committing to anything.
We're located at 8966 Glendale Milford Rd, Loveland, OH 45140, inside Fitness 3K Gym. There is on-site parking available directly at the gym. If you're coming from Montgomery, Madeira, or the surrounding Northeast Cincinnati area, we're an easy drive with convenient access off Glendale Milford Rd.
We're open Monday, Wednesday, and Friday from 8:30 AM to 4:00 PM, and Tuesday and Thursday from 12:00 PM to 7:00 PM. We're closed on weekends. If you're unsure about scheduling, just call or send a message and we'll find a time that works.
